Viewing Your Forms

If your responding organization has completed a verification on your behalf, follow the steps below to view it.

Step 1: Access Your Account

• Login to your account.

Step 2: Locate the Verification

• Click My Requests
• Locate the verification and click View Details

Step 3: Verification Responses

• Scroll down to the Verification Responses section to see responses for each question.
• If you disagree with any of the responses, please submit a Data Dispute.
